I'm gonna say that ATV roll I did a week or so ago. 75ft down a steep hill in front of the thing the entire time.

Any of the old 60's horror B movies might take that title from a technical perspective, but my absolute least favorite movie was The Tomb. It butchered one of my favorite Edgar Allan Poe stories (Ligeia) to the point where I was literally wondering if the writers had even read it. It wasn't an adaptation, all they did is take a couple names.
Lovecraft is a perennial favorite. His early works are highly derivative, though still good writing (the beginning of The Outsider reads almost identical to Poe's Berenice), but once he came into his own style, he was phenomenal. Rats in the Walls would easily be one of the top ten horror stories ever written imo. He did tend to be excessive in his use of vivid language, but it works.
